These Terms and Conditions (“Terms”) constitute a legally binding agreement between you (“Client,” “you,” or “your”) and Zen Functional Medicine, formerly Zen IV Therapy (“Company,” “we,” “us,” or “our”), a functional medicine clinic located at 5930 LBJ Freeway, Suite 370, Dallas, TX 75240.

By visiting our website at zenfunctionalmedicine.com, booking a consultation or service, or communicating with us via phone, SMS, email, or in person, you acknowledge that you have read, understood, and agree to be bound by these Terms and our Privacy Policy, which is incorporated herein by reference.

02 Description of Services

Zen Functional Medicine provides functional medicine services, including diagnostic testing, hormone, peptide, and IV nutrient therapies, and related wellness services (“Services”), provided by licensed healthcare professionals in our Dallas, Texas clinic.

Our Services include but are not limited to:

• Advanced diagnostic and laboratory testing to identify underlying causes
• Chronic fatigue and brain fog evaluation and support
• Hormone balancing
• Peptide therapy
• IV vitamin and nutrient infusion therapy
• Structural integration
• Massage therapy

All services are performed by licensed healthcare professionals and trained clinical staff. Service offerings are subject to change without notice. We reserve the right to decline service to any individual at our discretion for health or safety reasons.

04 Eligibility & Health Requirements

To receive our Services, you must:

• Be 18 years of age or older (minors require written parental or guardian consent)
• Complete a health intake form accurately and honestly before your first session
• Disclose all current medications, supplements, allergies, and pre-existing health conditions
• Disclose if you are pregnant or breastfeeding, as certain services may require clearance from your physician
• Not have contraindications to the requested service, as determined by our clinical staff

We reserve the right to decline or discontinue services to any client if, in the clinical judgment of our licensed staff, a service poses an unacceptable health risk. Our clinical staff may conduct a brief health screening prior to each session.

You are responsible for providing accurate and complete health information. Zen Functional Medicine shall not be liable for adverse events resulting from incomplete or inaccurate health disclosures.

05 Appointments, Cancellations & Refunds

Booking Appointments

Appointment requests may be submitted online at zenfunctionalmedicine.com or by calling (214) 807-1877. After you submit a request, a member of our team will contact you to confirm your appointment time and verify your insurance. Your appointment is confirmed upon receipt of a confirmation from us. We recommend booking in advance, as availability is limited.

Cancellation Policy

We ask that you provide at least 24 hours’ notice if you need to cancel or reschedule your appointment. Late cancellations (less than 24 hours’ notice) or no-shows may result in a cancellation fee. We reserve the right to implement and enforce cancellation fees at our discretion.

We reserve the right to cancel or reschedule appointments due to staff availability, equipment issues, or circumstances beyond our control. In such cases, we will contact you as soon as possible to reschedule.

Refund Policy

Prepaid services are generally non-refundable once a session has been administered. If you are dissatisfied with your experience, please contact us at (214) 807-1877. We will review your concern and work to find a reasonable resolution on a case-by-case basis. Package purchases that are unused may be subject to a partial refund at our discretion.

06 Payment Terms

Payment is due at the time of service unless prior arrangements have been made. We accept major credit cards, debit cards, and other payment methods as displayed at the time of booking or service.

Some services, such as office visits and laboratory testing, may be covered by your health insurance. Coverage varies by plan, and we will verify your benefits before your appointment. Other services, such as IV therapy and peptide therapy, are generally not covered by insurance. You are responsible for all charges not covered by your insurance, including copays, deductibles, and coinsurance.

All prices are subject to change without notice. Pricing in effect at the time of your appointment will apply. Any applicable taxes will be added to your total.

07 SMS Communications

About Our SMS Program

Zen Functional Medicine — Zen Functional Medicine is a functional medicine clinic located in Dallas, TX. When you opt in to our SMS program, you can expect to receive text messages related to: appointment reminders and confirmations, follow-up care check-ins, wellness tips and service updates, and promotional offers about our services (only if you have specifically opted in to marketing messages).

SMS Data Policy: No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. Information sharing to subcontractors in support services, such as customer service, is permitted. All other use case categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

How to Stop Receiving Messages

You can cancel the SMS service at any time. Just text “STOP” to (214) 807-1877. After you send the SMS message “STOP” to us, we will send you an SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us. If you want to join again, just sign up as you did the first time and we will start sending SMS messages to you again.

Carrier Liability

Carriers are not liable for delayed or undelivered messages.

As always, message and data rates may apply for any messages sent to you from us and to us from you. Message frequency varies per user based on interactions and appointment activity. If you have any questions about your text plan or data plan, it is best to contact your wireless provider.
